L'os compact ou os lamellaire ou Haversien constitue les parois de la diaphyse ou corticales. Cet os est composé de systèmes de Havers. Chaque système ou ostéon est formé :
- d'un canal central (canal de Havers) contenant du tissu conjonctif vascularisé, des ostéoblastes et des ostéoclastes ;
- de lamelles osseuses concentriques dans et entre lesquelles se disposent les ostéocytes.
Chez l'adulte on trouve des systèmes de Havers incomplets : les ostéoclastes digèrent la matrice osseuse tandis que les ostéoblastes synthétisent une nouvelle matrice. L'os est en perpétuel remaniement.
Ces canaux sont orientés selon le grand axe de l'os ; ils communiquent entre eux par de fins canaux tranversaux ou obliques (canaux de Volkmann) qui contiennent des éléments vasculo-nerveux. Sur une coupe d'os, on observe ainsi, sous le périoste, une couche osseuse lamellaire externe (système fondamental ou circonférentiel externe), plus profondément la masse des ostéons, puis une couche osseuse lamellaire interne (système fondamental ou circonférentiel interne) précédant la cavité médullaire.
